Youth Release

In order for your child to participate in WSC programs, you must agree to the Parent Authorization, Release and Indemnity.  Please read this release carefully.

PARENT AUTHORIZATION, RELEASE AND INDEMNITY

I/we undersigned parent(s) or guardian(s) of the above-named child approve of our child’s participation in any and all of the activities of the Weston Soccer Club, Inc. (the “Club”), during the upcoming indoor season and thereafter until I/we withdraw approval in writing; and, except if and as noted below, represent and warrant that our child has been examined by a physician and determined to be physically fit and able to participate.

We recognize that soccer programs have inherent dangers of participation, and hereby absolve and irrevocably release the Club, its directors, team managers, coaches and other officials and representatives, and all other persons connected with its programs, including referees and other players (collectively, the “Indemnitees”), of any responsibility or liability for accident or injury to our child while participating in any Club activity, and will indemnify and hold the Indemnitees harmless against any claim therefor.

I/we understand that it is the policy of the Club to request the presence of a child’s parent or guardian at all activities within or outside the Town of Weston attended by the child, and that neither the Club nor any other person connected with the Club assumes any responsibility for rendering emergency first aid in the event of any injury to or sickness of a child before, during or after any Club activity (nor to have a physician or other medical trained medical personnel present).  However, in the event that I/we happen not to be present at a particular event, I/we authorize the Club personnel present to administer any first aid to our child that they believe to be advisable and, if they believe it to be advisable, to transport our child to a hospital or other healthcare facility to receive medical treatment.

I/we agree to pay for the cost of any damage to Club or municipal property caused by acts of any family member.
