Adult Release

In order to participate in WSC programs, adults must agree to the Adult Authorization, Release and Indemnity.  Please read this release carefully.

AUTHORIZATION, RELEASE AND INDEMNITY

I request the opportunity to participate in the activities of the Weston Soccer Club, Inc. (the “WSC”), during the upcoming season and thereafter until I withdraw my approval in writing; and, I represent and warrant that I have been examined by a physician and determined to be physically fit and able to participate.

I recognize that soccer programs have inherent dangers of participation, and hereby absolve and irrevocably release the WSC, its directors, team managers, coaches and other officials and representatives, and all other persons connected with its programs, including referees and other players and the Town of Weston, CT (collectively, the “Indemnitees”), of any responsibility or liability for accident or injury I might suffer while participating in any WSC activity, and will indemnify and hold the Indemnitees harmless against any claim therefor. 

I understand that neither the WSC nor any other person connected with the Club assumes any responsibility for rendering emergency first aid in the event of any injury to or sickness I might suffer before, during or after any WSC activity (nor to have a physician or other medical trained medical personnel present).  However, I authorize WSC personnel present to administer any first aid to me that they believe to be advisable and, if they believe it to be advisable, to transport me to a hospital or other healthcare facility to receive medical treatment. 

Further, in the event that I damage any WSC or municipal property during a WSC activity, I agree to promptly pay for the cost of such damage caused by my act.
