Required Forms

1. A completed Pass Facsimile form (Click here to download) with a 1 inch X 1 inch photo affixed and with the PLAYER's signature.
    
2. MEDICAL RELEASE FORM -- Download and complete.
    
3. If the player WAS NOT on a Weston travel roster since Spring 2008, then bring COPY of Birth Certificate or Passport .; Do not bring originals, we accept only copies.
    
4. If the player was NOT born in the US and WAS NOT on a Weston travel roster since Spring 2008, then the following is required:
   • • • Proof of US citizenship or the following:
          
       • If Child is 11 or younger, then need completed Proof of Entry Form with item of proof which may be certified U9/U10 roster.
         * Once this is done, no other forms will be required as the child ages.
       • If Child is 12-16, then need only Clearance Waiver Form.
       • If Child is 17 or older, then need Clearance Request Form.
         * If has proof of living in US prior to 12, then also need completed Proof of Entry Form with item of proof.
