Required Paperwork  

 

The following is required in order to place a player on a certified CJSA roster. They may be mailed to: Marcia Hamelin, 18 Echo Hill Road. Weston, CT.

• A completed Pass Facsimile (click to download) form with a 1 inch X 1 inch photo affixed and with the PLAYER's signature. (Please note that this photo may be a photocopy on printer paper as long as the player's face is clearly visible.)
   
• A COPY of Birth Certificate (not hospital certificate) or Passport unless player was on a Weston Travel team at some time since Spring 2008 .; Do not bring or mail originals, we accept only copies.
   
• If player was NOT born in US and was NOT on a Weston travel roster since Spring 2008, then contact for additional mandatory paperwork.
